Please complete this form to apply for Youth Partnership for Health (YPH). Members meet one Saturday per month from September to May. This application is for the September 2023 to May 2024 season.

Members of YPH are paid $75 per meeting ($25/hour) in addition to other benefits. After completion and review of all applications, some of you will be contacted and asked to attend an interview on April 8th. This interview will be a virtual interview.
